Treat it as a public website with an enquiry inbox, not a clinical system. Appointment requests and messages carry a name, contact details and whatever the patient chooses to write, so it is not the place for clinical records or anything you would hold in a patient management system. Keep clinical notes where they belong and use this for the front door.
Treatments can be listed per practitioner — "Physiotherapy with a senior practitioner" as its own entry, for example — and backgrounds and credentials go on your About page, which is yours to write. There is no separate staff directory with its own profiles.
Yes, through a review link of your own, and nothing is published until you approve it. Be careful about what you invite in a clinical setting — a review that describes someone's treatment can be a confidentiality problem even when they wrote it themselves.
